Job DescriptionJob Description:This RN works directly with Medicaid & Medicare patients in the community on the MSHO program, assessing them in their homes and helping connect them with medical care and services. Will chart and document from home when not performing face to face assessments. Assessments are questionnaire and information based rather than physical assessments. The ideal candidate for this position works independently, is comfortable in others’ homes, is organized, manages time well, and is willing to ask questions when faced with unfamiliar situations. This position is a hybrid remote role that requires face to face assessments, so the candidate must reside in the service area. Essentia provides needed technology, and the candidate is responsible for reliable internet service and an office space, as well as reliable transportation. Mileage is reimbursed at the current IRS rate, and the position is salaried. This employee serves as a float position throughout the whole state of Minnesota.
