Department Details As a Home Health RN Case Manager for Good Samaritan Society Home Care of St. Peter, you will work to integrate and develop the service plans for our clients while also managing their care and providing guidance to staff. The RN Case Manager opportunity will be scheduled at 30 hours per week, but could build up to 40 weekly. Monday - Friday generally between 8am to 430pm. One on-call rotation every 5-6 weeks, phones roll over from 430pm to 8am weekdays and 24hrs on the weekend. 1-2 holidays per year on-call rotation. Mileage reimbursement at the federal rate.
