This is a float-travel position that includes supporting the following areas: Duluth, Grand Rapids, Hibbing, Brainerd, St. Cloud and Metro/Twin Cities.Purpose:A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Executive Director /Clinical Manager and Therapy Manager. Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s). Participates in patient, caregiver, and family counseling and caregiver education. Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care. Skilled Professional Staff are expected to contribute to all phases of the QAPI program and participate in ongoing interdisciplinary assessments of patients. Supervises Physical Therapy Assistants and Home Health Aides as needed.Reports toDirectly to Executive DirectorIndirectly to Therapy Manager/Regional Director of Therapy ServicesStaff Who Directly Report:PTA’sClinical Duties and Responsibilities:Develops a patient centered plan of care performing initial and follow up assessments assisting physician with determining level of patients function and revising as necessary.Obtains orders and provides all physical therapy services according to physician orders.Informs the attending physician and other team members of changes in the patient's condition and needs.Involves patient/family in development and updating of plan of care and provides education as needed.Interacts with other clinical team members for coordination of patient care through SOC conferences, case conferences and/or with clinical manager evaluation findings.Initial evaluation encompasses a holistic approach with review of medical history, body systems, vital assessments, objective testing procedures, creates problem list, assessment and establishes measurable goals.Consistently prepares, completes, and submits all documentation in EMR within 24 hours.Re-evaluates needs of patient/family at least every 30 days and documents comparison between prior and status and provides an analysis of results in assessment.Documents discharge summary upon completion of therapy services.Completes routine visit notes that document specific treatment performed, patient’s response to treatment and progress or regress towards goals.Performs therapy Admissions / OASIS SOC / ROC and Recertifications every 60 days routinely and submits completed documentation within 24 hours.Consults with attending physician to determine whether treatment plan should be continued, changed, or terminated.Attends staff meetings regularly and participates in patient care conferences as scheduled.Immediately reports to RN, Clinical Director/Clinical Manager any patients’ incidents/variances or complaints and documents according to policy.Participates and contributes to ongoing interdisciplinary assessment(s) of patients.Attends and participates in in-service education programs as assigned.Reconciliation of MedicationsFollows established plan of care by minimizing missed visits.Evaluates the home environment for safety hazards and makes recommendations. Identifies patient needs for services from other disciplines and consults with other professionals.Customer service focus by informing patients in advance of visit scheduled time, notifying if late, being courteous, respectful, and friendly.Professional Duties and ResponsibilitiesProvides physical therapy consistent with the American Physical Therapy Association Code of Ethics and Standards of Practice.Maintains sound privacy and security practices and prevents privacy or security breaches. If a breach occurs, corrective action will be taken.Fulltime therapist is responsible to meet weekly productivity and be available during operational hours of the organization to service patients.When position has productivity weekly requirement, clinician consistently takes responsibility for monitoring and meeting weekly productivity expectation. Will proactively seek out assistance from branch.At times may need to accept patient assignments outside of normally assigned geographic area to meet productivity threshold and or prevent patient service issues.Participates in monthly weekend rotation (FT required) and Holiday Rotation (FT required) as dictated by branch needs.Adheres to Agency standards and consistently interprets and accurately performs all assigned responsibilities.Demonstrates competent performances of technical skills according to established procedures.Consistently works with Executive Director/Clinical Manager to ensure proper utilization of services.Supervision Works with PTA on regular basis as dictated by branch needs.Makes recommendations concerning changes in the Plan of Care, modifies the Plan of Care in keeping with individual needs and functional progress of the patient to PTASupervision requirements followed per state practice act and or federal regulations.Regularly communicates with PTA via phone, in person, secure messaging or some format regarding patients under their care no less than weekly.
