Schedule: Must be available to service the customer and take appointments between the hours of 8:00am - 6:00pm, Monday - Saturday, with last appointment availability starting at 6:00pm on a rotating schedule, 5 day work week including weekends if your rotation shift falls on a weekend. Must be available to work 2-3 Saturdays per month (50% Saturdays per month).

The RN Case Manager opportunity will be scheduled at 30 hours per week, but could build up to 40 weekly. Monday - Friday generally between 8am to 430pm. One on-call rotation every 5-6 weeks, phones roll over from 430pm to 8am weekdays and 24hrs on the weekend. 1-2 holidays per year on-call rotation. Mileage reimbursement at the federal rate. Most visits are in a 30 mile radius of St. Peter. Visits include admissions, recertifications, resumptions, routine, and discharges. Able to work with growth and development needs of client populations in clinical area. May work with clients in all age groups. Ability to communicate with patients/residents, family members and others on the health care team. Coordinates, facilitates, and negotiates with others. Monitors, evaluates and trends patient/resident responses utilizing structured care methodologies. Fiscally responsible. Controls variances and duplication. Qualifications Bachelor’s degree in nursing preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Minimum of one to two years of professional nursing experience in a clinic care setting preferred. In-depth knowledge of specialty area preferred. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required.
