How You Will Make a Difference

As the Aide, you will:
Work in collaboration with the RN to fulfill the defined patient-specific Aide care planCollaborate with the care team as an extension of nursing or therapy services to ensure all patient needs are fully addressedDeliver hands-on personal care and supportive services to assist with safe transfers and ambulationThoroughly document care delivery daily in our Electronic Medical Record systemContribute to a culture of caring through individual accountability and teamwork

Experience to Deliver on our Mission:
High school diploma or equivalent preferredCertification required for State-specific certified aide certificationState licensure or a competency evaluation program, valid state driver’s license and auto liability insuranceOne year experience as a Home Health or Hospice Aide or Nursing Assistant in a hospital, nursing home, or home health agency. Home health experience preferred
