They must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience. Completes comprehensive assessment/examination/evaluation, develops/modifies plan of care in conjunction with patient/caregiver and physician, provides treatment visits, and provides discharge planning to these patients to facilitate patient/caregiver engagement for achievement of patient goals, clinical outcomes and positive patient experience.
