The Orthopaedic Navigation Services Coordinator-RN is responsible for improving navigation workflow, identifying opportunities for improvement and the coordination of orthopaedic and spine patient care.This position supports team members in alignment with HHC’s vision, core values and leadership behaviors.This is an experienced and advanced level position that provides clinical expertise to members of the departmentJob Responsibilities:Serves as ambassador of the H3W leadership behaviors.Provides coaching and training to new employees or for new tasks to existing employeesAdvances the profession of nursing by contributing to the clinical development of othersServes as a resource to colleagues on issues/questions and provides guidanceCollaborates with members of the inter-professional healthcare team to assess learning needs, monitor clinical competency and support action plansCoaches for improved performance by providing appropriate feedbackCommunicates practice/programmatic changes to the multidisciplinary teamPromotes change management initiatives to drive desired outcomesPartners with organizational leadership team to facilitate improvements and assist in the measurement of outcomesMaintains a positive environmentProvides appropriate feedback to program manager regarding any concern/issues that may ariseAssists with onboarding and training of new staff.Acts as the subject matter expert (SME) in a designated clinical program or population.Providesguidance and expertise to colleagues and patients within specialized skillset(s)Researches and recommendsbest practices for implementationWorks with team tostandardize care and create treatment guidelinesServes as a resource and role model for high reliability organization (HRO) focus and integrates HRO initiatives into practiceLeads special interest group and/or councils, to ensure excellence in care for a specific patient population.Assists with the operations and coordination of the department or site as assigned by the program manager to include but not limited to:·Assisting with department huddles·Managing drivers·Implementing countermeasuresDeveloping navigator work schedule.Assists with quality control, data collection and quality assurance for regulatory compliance and accreditation. Identifies opportunities for improvement and recommends process improvements utilizing operating model.Facilitates the patient experience across and tracks patients within the continuum of care. Collaborates with the multidisciplinary team to accomplish consistent systems of care.Performs navigation duties.
