This responsibility includes participation in the screening of employment applicants, orientation, education, and evaluation of specialty nursing staff, and ensuring that quality patient care is delivered efficiently. A valid NY RN license is required. Diabetes Educator Certification required. Must demonstrate ongoing participation in educational opportunities prepared for National Certification Board Diabetes Educators exam every five years. Ability to travel and valid NYS driver’s license required. Must demonstrate solid interpersonal, organizational and time management skills. Basic computer skills are necessary.RESPONSIBILITIESConsultative - Provides consultation to the patients/PCG staff who is participating in the delivery of care for management to the high-risk Diabetes clients, and to the generalist whom is caring for the less complicated Diabetes Patient.Operations - Participates in the development and maintenance of current operational policies and procedures related to Diabetes programs.Training - Responsible for development and implementation of orientation, education, and training activities for agency regarding Diabetes management of persons/PCGQuality Management - Collaborates with the Quality Management staff to design, monitor, and evaluate the continuous quality improvement process, and risk management components of the clinical program. Conferences, joint visits with, and convenes specialty team on a monthly basis to evaluate and update program.EDUCATION:LICENSES / CERTIFICATIONS:PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations. PAY RANGE:$67,500.00 - $90,000.00CITY:RochesterPOSTAL CODE:14607The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
