JOB SUMMARY: The licensed practical nurse will coordinate resident care and designate assignments to other staff members so that optimum resident care is provided. This position is responsible for reporting any changes in residents’ health condition to physicians and supervisors. Direct the day-to-day functions of nursing assistant and other staff being supervisedGive direct nursing care to residentsVisit all assigned residents at regular intervals during tour of dutySupervise feeding of residents as necessaryNotify physician/physician assistant/nurse practitioner when resident’s condition warrants Perform administrative duties, such as documenting of:Nurses’ notesAssessmentsPhysician/Nurse Practitioner/ Physician’s Assistant ordersDischarge plansDietary changesReportsMiscellaneous forms, charts, etc.Chart pertinent facts and information in a professional, clear and comprehensive mannerAdmit, transfer and discharge resident according to policyRecognize and respond to changes in residents’ conditions and document observations, interventions and outcomesOrder from pharmacy, prepare, administer and document medications and treatments as ordered by physician/nurse practitioner/physician’s assistantEnsure that physician/physician assistant/nurse practitioner sign off on all verbal orders and document processConduct and/or monitor prescribed treatments, such as dressing changes, drawing of blood, catheter changes, NG tube insertion and removal, laboratory work, etc.Give and receive condition report from the on-coming and off-going shiftPerform emergency procedures, such as cardiopulmonary resuscitation, and in the event of death, notify family and appropriate other partiesRecognize, intervene, report and document accidents and incidents when they occurMonitor and update care plans on each shift as appropriateSupervise and participate in fire safety and other disaster drills and be prepared to carry out duties in the event on an actual eventRecognize, intervene and/or report complaints and grievances from resident’s, families, visitors and employeesMonitor the facility for possible health and safety hazardsMaintain privacy and confidentiality of records, conditions and other information relating to residents, employees and facilityFollow the established universal precautions and isolation proceduresUpdate daily census report and submit as requiredAttend and participate in orientation programs, on-going training and educational classesCheck inventory and request equipment, supplies, etc.Assist in updates of written policies and procedures of routine nursing careParticipate in surveys made by authorized government agenciesMonitor and participate in daily clean-up of the nurses’ stationPerform other related duties as assigned by supervisor JOB REQUIREMENTS: Communication skills and active listeningPossess good interpersonal skills while dealing with emotional or difficult situations.
