In collaboration with the administrative leadership, will implement and maintain the program and provide overall direction and monitoring of patient outcomes and core measures for the heart failure patient. 7. Responsible for implementing program changes to ensure program success as patient’s or physician needs change and/or any regulations associated with the program change. 8. Assist in making sure that community outreach and patient education events related to the early detection and prevention of heart disease are met, with a minimum of quarterly participation. 9. Serves as a connection between the public and the facility and will commit to working and promoting the Cardiology and Health system structure 10. Supports and provides transition of care best practices including but not limited to referrals, follow up appointments and calls, including patient and family education.
