Key Responsibilities Perform initial and ongoing patient assessments, including OASIS documentation.Develop and implement tailored treatment plans in consultation with physicians, adjusting as needed.Collaborate with the interdisciplinary care team to optimize patient outcomes.Provide education and support to patients and families on therapy programs.Supervise Physical Therapist Assistants every 30 days.Communicate patient progress and changes with physicians and care teams.Document all visits and progress notes on the same day of service.
