This position requires the expertise to provide empathetic and evidence-based care. Responsibilities: provide diabetes self-management training and support in the outpatient and inpatient setting, individual dietary and health behavior counseling in the weight management clinic, and participation in diabetes-related and weight management programs.Responsibilities:Assesses patients nutritional status and needs. Develops and implements care plans, monitors patients' progress, and updates interventions. Completes thorough and timely documentation of patient sessions in the electronic health record.Performs nutrition focused physical examinations and collaborates with the physicians on malnutrition diagnoses when appropriate.Collaborates with physicians, nursing, pharmacy and other healthcare team members and departments to determine, coordinate, and provide medical nutrition therapy.
