General Summary of PositionFacilitates the inpatient admission of appropriate rehabilitation patients to MedStar National Rehabilitation Network (MNRN) by obtaining accurate clinical data through performing on-site and record/telephone patient evaluations. Individuals in this role are responsible for educating the referring staff of community hospitals, patients, family members and external payers regarding the effective and efficient utilization of MNRN program services and resources. Primary Duties and ResponsibilitiesPerforms on site, record and telephone preadmission screening/clinical assessment to determine appropriateness of referral and medical stability for inpatient acute rehabilitation program.Completes assessment/evaluation within 24 hours or in one working day of the referral (documents exceptions within the electronic medical record).Reviews clinical information in patient's medical record and performs physical assessment of the patient to verify/confirm information in medical record.Interviews referring healthcare team members, patient/family and prepares a summary report at time of initial on-site evaluation.Provides preliminary recommendation to referral source regarding appropriate level of care of potential patients.Reviews clinical data/assessment with MNRN manager and/or physician for determination of medical stability and appropriateness for admission and notifies referral source of MNRN decision.Educates patient, families or significant others, referral sources, hospital personnel, external payers and case managers regarding inpatient acute rehabilitation program service and benefits, admission criteria, bed availability, day treatment programs, outpatient centers/programs, and basic knowledge of third party reimbursement.Maintains marketing plans for each assigned hospital. Maintains a thorough knowledge of MNRN facilities, products, and services. Develops and maintains ongoing relationships with referral sources. Prepares/submits marketing reports and all necessary data for accurate maintenance of statistics.Participates in formal marketing programs as directed, including tours for family members and referral sources.Participates in service area activities, quality improvement projects, MNRN program steering committees and attends Rehab-focused Grand Rounds on the hospital campus annually.Provides feedback on admitted patients' progress at MNRN to referral sources as requested.Serves as Rehabilitation Liaison consultant/resource for referring hospital personnel.Provides recommendation for alternative level of care when patient needs cannot be met at M NRN.Submits preliminary treatment plan, estimated length of stay and rehabilitation goals required for precertification process.Communicates verbally with appropriate MNRN staff regarding special needs" e.g. equipment, social issues, etc.Coordinate CEU/formal and informal educational opportunities annually for internal/external customers according to expectations set by Admissions Manager.Participates in shadowing opportunities on the hospital campus; assists in piloting new technology, or leads an engagement activity for team members according to expectations set by Admissions Manager.
